    Jennifer Young, RN, BSN, is the Lead Nurse for the Sedro-Woolley School District. Ms. Young's office is located at Lyman Elementary and she is available Monday - Friday during school hours. She can be reached at 360-855-3635.

    Immunizations: Students cannot attend school unless immunization information is current.

    Vaccines

    Diphtheria, Tetanus, Pertussis (DTap/DT/Td)Five doses (four doses only IF the fourth dose is given on or after the fourth birthday).

    TdapOne dose required for 6th - 12th grade AND on or after 11 years of age and IF student has been at least 5 years since last DTap, DT or Td.

    Polio (OPV/IPV)Four doses (three doses only IF the third dose is given on or after the fourth birthday).

    Measles, Mumps, Rubella (MMR)Two doses, first dose on or after the first birthday, the second dose at least 28 days later.

    Hepatitis BThree doses before entering school AND third dose after six months of age.

    Varicella (Chickenpox)Two doses, required for K-8th grade. First dose on or after the first birthday; and second dose at least three months later OR healthcare provider verifies disease.
