 is the District Nurse for the Sedro-Woolley School District. Her office is at Central School and is available Monday-Friday during school hour

Students cannot attend school unless immunization information is current

Washington State Immunization Law
(RCW 28A.210.118, 1990 and WAC 246.100.166, 1991)

Vaccine  How many doses are required
Diphtheria, Tetanus,
Pertussis (DTap/DT/Td)
Five doses  (4 does only IF 4th dose is given on or
after the 4th birthday)
One dose required for 6th-12th  grade AND on or after 11 years of age and
IF student is has been at least 5 years since last DTaP, DT or Td.
Polio (OPV/IPV) 

Four doses (3 doses only IF 3rd dose is given on or after the

4th birthday).

Measles, Mumps, Rubella(MMR)
Two doses, first dose on or after the 1st birthday,
second dose at least 28 days later.
Hepatitis B Three doses before entering school AND 3rd dose after 6 months of age.
Varicella (Chickenpox)
Two doses, required for K-8th grade. First dose on or after the 1st birthday;
and second dose at least 3 months later OR Healthcare provider verifies disease.
